I would like to register my child and/or children for Standardized Testing with BEECH Homeschool Community.

I understand BEECH Homeschool Community is offering to test my child/children with the Stanford/OLSAT Standardized Test.

I understand that the OLSAT portion of the exam is optional.  

I understand and agree that I am responsible for PURCHASING and ORDERING my child's/children's test from BJUPress.com no later than April 25, 2019.  

I understand that ordering and purchase instructions will be sent to me via email once BEECH has accepted my child's registration.  I agree that I will NOT order the test before instructions are received from BEECH.

I understand that testing will take place only on May 28, 29 and 30th of 2019. 

 I further understand that I should arrive no later than 9:45 a.m. and that testing will begin promptly at 10 a.m. whether or not my child or children are there. 

I understand and agree that I need to arrive by 9:45 a.m. and I will pick my child/children up NO later than 1 p.m. unless I have made prior arrangements. There will be a $10 charge for every 10 minutes I am late.

I also understand and agree that if my child does not behave or is disruptive, my child will be removed from the testing room immediately so as not to disturb other children testing and I will be called to pick the child up immediately.  I agree,  If the child needs to be removed - this child will not be able to finish that portion of the exam.  If I have other children testing that are behaving appropriately they will be allowed to continue testing.

I understand and agree that my child/children are responsible for their own belongings.  I understand and agree that my child/children bring these items at their own risk.  I understand that all efforts will be made to find any lost items but I agree that BEECH Homeschool Community and its members will not be held liable for any lost or broken items of my child/children.

I understand and agree that available testing spots are on a first come, first serve basis with first priority given to BEECH Members and then to Children in Mandatory Testing Grades.  As such, I understand and agree that registering here does NOT guarantee any of my children a test spot.  

I understand that BEECH Homeschool Community is offering Group Testing Only and my child/children will be tested with the appropriate grade and other children in a group setting.  I further understand that private testing with BEECH Homeschool Community is NOT Available.

I understand that I MUST send my child/children with the following items in order to test and I agree to send each of my children with the following items every day during testing:

1. At least 3 sharpened No. 2 pencils
2. Blank Scratch Paper
3. A pink eraser
4. Bottled Water
5. Appropriate Snacks and/or Lunch
6. Something quiet to work on or read for when my child finishes the test section so as not to disturb other testers.

I understand and agree that no electronic devices will be allowed in the testing area.  They must remain off and inside a purse or backpack.

I understand and agree that BEECH Homeschool Community is charging a $25 proctoring/tester fee for my child, $15 for a 2nd child testing and $5 for each additional child to test payable only via Paypal to [email protected]  

I understand and agree that the Tester/Proctoring fee includes the cost of postage to return the test and answer sheets to BJUpress for grading.  

I understand and agree that I WILL INSTRUCT my child or children NOT to write in the test booklet or damage it in any way.

I further understand and agree that if my child damages the test booklet in anyway - that I will pay for the damage and may be liable for further charges by BJUPress.  I agree that BEECH Homeschool Community will not be held responsible for any damage that my child does to the test booklet.

I understand and agree that my child's registration will NOT be accepted until the proctoring/tester fee is paid.  (Beech members test for free).  

I understand and agree that BEECH will Fully REFUND the registration fee for any child that is NOT accepted for testing with BEECH.  

I understand and agree that payment and filling out this form does not guarantee my child a testing spot with BEECH.  My child's spot is only confirmed once I receive an email confirming each registered child's spot.

I understand and agree that BEECH will be responsible for returning the tests and answer sheets to BJUpress, postage is included in the fee I pay to BEECH

I understand and agree that once my child's test spot is confirmed by BEECH, should I decide NOT to test with BEECH my fee is not refundable.  I may change my mind about testing anytime prior to having my spot confirmed and receive a full refund minus a $2 processing fee.

I understand and agree that I MUST order my child's test from BJUPress NO LATER than APRIL 25th per the instructions I am given by BEECH Homeschool Community once my child's registration is accepted.  I understand if I order after April 25th that my test may not arrive in time for the BEECH Test dates.  BJUPress does offer expedited shipping but it would be my responsiblity to see if expedited shipping will get the test here in time.  I understand that expedited shipping fees can be $30 or more.  I understand and agree that Proctoring/Testing refunds will not be given to those whose tests do not arrive in time for testing.

I understand and agree that I am solely responsible for ordering from BJUPress the CORRECT Grade and Level Stanford exam or Stanford/OLSAT exam for EACH Child accepted for testing with BEECH.  I understand and agree that BEECH is not responsible if I order the wrong tests and my child/children are not able to test and proctoring/tester fees will not be refunded.

I understand that BEECH Homeschool Community is offering standardized testing as a service to the homeschool community and will do their best to accommodate the needs of my child.  However, since I am NOT a member of BEECH Homeschool Community, I understand and agree that if my child has a special need or learning disability that BEECH does not feel they can handle adequately on their own then my child may not be able to test with BEECH Homeschool Community or I, as the parent, may be required to stay and assist with my child in order for the child to test.  

I understand and agree that BEECH is not set up for private testing and if my child needs/requires private, one on one testing or other special requirements (example: every question being read to the child, oral answers provided instead of written, etc), I understand and agree that BEECH is not able to offer those kinds of accommodations.  Testing is only offered in a group setting with no special requirements or accommodations being available.

Waiver of Liability, Assumption of Risks, and Indemnity Agreement

WAIVER: In consideration of my child or ward or spouse or me being permitted to participate in any way in any and all activities of B.E.E.C.H.  Homeschool  Community (“BEECH”) at the residence of any Member or at any and all locations where such BEECH activities take place (collectively, the “Activities”), I, for myself, my spouse, my child and/or ward, my heirs, personal representatives or assigns, do hereby covenant not to sue, and release, waive, and discharge, BEECH, its directors, officers, staff members, volunteers, advisors, property owners, and/or agents, from liability and of and from any and all claims including the negligence or default of BEECH, its directors, officers, members, volunteers, advisors, property owners, and/or agents, resulting in personal injury, accidents or illnesses (including death), and property loss arising from, but not limited to, participation in the Activities.

ASSUMPTION OF RISKS: Participation in the Activities carries with it certain inherent risks that cannot be eliminated regardless of the care taken to avoid injuries. The specific risks vary from one activity to another, but the risks range from (1) minor injuries such as scratches, bruises, and sprains, (2) major injuries such as eye injury or loss of sight, joint or back injuries, broken bones or teeth, asthma or heart attacks, and concussions, to (3) catastrophic injuries including paralysis and death.

I have read the previous paragraphs and I know, understand, and appreciate these and other risks that are inherent in the Activities, which may include without limitation parties, picnics, BBQs, hiking, swimming, camping, field day, field trips, museums, plays, parks, recitals,  parties, practices or other such activities.   I hereby assert that my participation and/or the participation of my spouse, my children or wards is voluntary and that I knowingly assume and accept all risks. I also understand that I may participate in activities that are not sponsored or supported by BEECH but are private activities of Members and that I assume all risks associated with such private activities (“Non-BEECH Activities”).

INDEMNIFICATION AND HOLD HARMLESS: I also agree to INDEMNIFY AND HOLD BEECH, its directors, officers,  members, volunteers, advisors, property owners, and/or agents, HARMLESS from any and all claims, actions, suits, procedures, costs, expenses, damages and liabilities, including attorneys’ fees, brought as a result of my involvement in the Activities or Non-BEECH Activities or both, and/or the involvement of my spouse, a minor child or ward of mine in the Activities or Non-BEECH  Activities or both, in any location where Activities or Non-BEECH Activities or both are conducted, and of and from all claims which may hereafter develop or accrue to them on account of injury, loss or damage, which may be suffered by said minor or to any property, because of any matter, thing, or condition, negligence or default whatsoever, and to reimburse them for any such expenses incurred.

SEVERABILITY: The undersigned further expressly agrees that the foregoing waiver, and assumption of risks, indemnification and hold harmless, agreement is intended to be as broad and inclusive as is permitted by the law of the State of Georgia, and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

ACKNOWLEDGMENT OF UNDERSTANDING: I have read this Wavier of Liability, Assumption of Risks, and Indemnity Agreement, fully understand its terms, and understand that I am giving up substantial rights, including my right to sue. I acknowledge that I am signing this agreement freely and voluntarily, and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.

I further agree to accompany or have another adult accompany my minor child at all times while they are involved in any Activities on the premises of any Member, or at any other location, and acknowledge that I am fully and totally responsible for the above child at all times while he or she is participating in any Activities at BEECH or at the homes of Members of BEECH or at any other location.
